These toolkits were developed by Nemours Children’s Health as part of its Exploring the Roles & Functions of Health Systems within Population Health Integrator Networks Initiative, with support from The Kresge Foundation. The initiative focused on understanding how health care institutions and their partners share responsibility for integrative activities, which are the governance, oversight, and administrative functions that enable population health networks to carry out tasks and strategies related to the network’s shared population health goals.
For networks to move the needle on population health outcomes and foster healthy, thriving communities, these tools will help networks prioritize which integrative activities to focus on and how to put those activities in action.
